laser 4000 width measure fail

I am having problems with what seems to be random width eye measure failure. I have replaced the sensor. The error pops up when the sensor is lit and appears to be aimed properly also the input 22 is lit. Looking for suggestions on what to check next.

I replaced an eye couple weeks of go and the eye was bad from the box drove me nuts
so i figure let me try another one for some reasone
and sure enough the new one was bad
