The 120VAC should go to the input of the transformer. The output of that transformer is 5V and should go to the terminals on the board. The various messages are played by different combinations of 25VAC inputs for the (1), (2), (4), (8) and (16) inputs (plus common).

It's possible the transformer is providing too much voltage. It's pretty standard that most electronic components should work + = 10%, so that would put it on the edge. That LED light should also illuminate if you press that test switch.
