Controller for Ultimate EM2 door opener???

I know this is probably going to end up being a wild goose chase, but I thought I would give it a shot. I bought one of these door openers at the show in 2018. I had it installed before last winter. It worked great, but this year it wouldn't come to life. The smoke has been released from the controller. Of course, Ultimate has gone by the wayside. Anyone have any suggestions on where to get this thing? Other than a # written on a white label with an ink pen, I don't see any identifiers. Help, please...

IB if you do track it down please share your source. We have four of those operators and will probably need one eventually.

FYI... if you ever have trouble with the motor brake my brother has successfully sourced those from two places. China, really cheap BUT you need buy a pallet full. Or some some company in Canada, expensive but can buy just one.
