Diagnosing chain slipping off sprocket on Belanger dura trans XD conveyor

Hello!

We have an old Belanger dura trans XD conveyor, what could be causing the chain to slip off the sprocket?

- chain tension from the pressurized air cylinders is good
- the entire take-up assembly seems to be turning/tilting to one-side before the slip occurs
- bearings seem to be okay, no perpendicular sprocket movement under force (we removed the entire assembly and inspected it however our knowledge and experience is extremely limited)
- frame holding up the sprocket assembly has been lubricated for the wheels to move on

lots of the equipment is considerably old and worn...
what would you check next?
where would you start?

Notice the gap between support wheels and the frame in the photo, is there meant to be that much space for perpendicular movement of the assembly?

The chain itself being worn, and or worn sprockets and or the sprocket assembly, and or sprocket bearings. also, there are two tensioning air cylinders one may not be working. In summary get an experienced service tech familiar with Belanger. He can better diagnose what has to be fixed today and what can wait. The Dura Trans is a great conveyor and worth fixing.

Thank you for your response, we had a local carwash mechanic come in and found out it was actually one of the bearings that had busted.
Experience goes a long way, we are grateful to have found some local help!
Conveyor is fixed and running well again.
