The first trip of the day and the last trip home are usually not deductible but any driving in between those trips are. I have also seen where all trips can be deductible if you are on call 24 hours a day. Your accountant could give you the best advise for your situation.

If you have a legit office at your home, then all the miles are deductible, otherwise, the first and last aren't.

I can deduct my mileage "to and from" because I cross a county line.
